A Housing Partner That Makes
Placement Easier

Precision Phase Solutions provides clean, safe, stable, housing-only shared living opportunities for independent adults. We give referral professionals a clear process, defined requirements, responsive communication, and a dependable resource for housing inquiries.

PPS provides housing only. We do not provide assisted living, treatment, supervision, clinical care, personal care, or case management.

Professional housing communication

Clear Housing Communication From
Referral Through Move-In

Referral professionals need housing resources that respond promptly, explain requirements clearly, and communicate honestly about availability and next steps. PPS is designed to reduce uncertainty and make the housing-referral process easier to manage.

Clear Referral Requirements

Partners receive straightforward information about who PPS serves, what documents may be needed, and what residents are expected to manage independently.

Prompt Referral Confirmation

PPS confirms when a referral or housing inquiry has been received and identifies missing information when necessary.

Controlled Email Updates

Authorized referral contacts receive referral-status updates by email. PPS does not require partners to access a public tracking portal.

Defined Next Steps

Partners receive clear communication regarding availability, information review, documentation, housing requirements, and move-in coordination.

Honest Housing Communication

PPS communicates when housing is unavailable, when additional information is needed, or when the available shared-home arrangement may not be appropriate.

WHO FITS PPS

Who May Be a Good Fit for PPS Housing

PPS housing may be appropriate for adults who are prepared to live independently, meet their financial responsibilities, follow the Resident Housing Agreement, and contribute respectfully to a shared residential environment.

  • Veterans and seniors who can live independently
  • Working adults seeking stable shared housing
  • Adults receiving SSI, SSDI, or VA benefits
  • Low-income adults with a sustainable payment plan
  • Adults reestablishing housing stability
  • Adults leaving care settings who are ready for independent living

Note: A referral does not guarantee housing. Placement depends on current availability, required information, financial readiness, and whether the shared-housing arrangement appears appropriate.

Before Referring, Confirm the Person Can:

  • Live independently without PPS supervision
  • Manage personal care and daily routines
  • Manage medications independently
  • Communicate basic housing needs
  • Follow written housing expectations
  • Respect other residents and shared spaces
  • Maintain bedroom and common-area cleanliness
  • Pay agreed housing charges
  • Follow guest and occupancy requirements
  • Avoid illegal, threatening, or seriously disruptive activity
  • Arrange transportation independently
  • Arrange outside healthcare or support services independently
  • Participate in the housing-documentation and move-in process

Important: PPS does not determine whether a person is medically or clinically ready for discharge. The referring organization remains responsible for its own discharge, service, care, and safety decisions.

REFERRAL TIMELINE

A Clear Referral Process

Our timeline is designed to give you clarity and responsiveness at every step—without the frustration of monitoring another referral portal. We provide controlled email updates and clear requirements so you know exactly where the housing request stands.

01

Submit the Referral

An authorized referral contact submits the PPS referral form and available housing information.

02

Receive Confirmation

PPS confirms receipt of the referral through controlled email communication.

03

Complete Missing Information

PPS requests additional documents or details when the referral cannot be reviewed as submitted.

04

Review Availability and Fit

PPS reviews current housing availability, requested timing, financial readiness, independent-living requirements, and the available shared-home arrangement.

05

Coordinate Next Steps

When an appropriate housing opportunity is available, PPS communicates documentation, payment, orientation, and move-in requirements.

If housing is not currently available, PPS may provide an availability update or explain possible next steps.

Referral Updates Without Another
Portal to Manage

PPS maintains private internal referral records and sends authorized referral partners controlled email updates. Partners are not required to create a public account or monitor a separate referral-status website.

NEED-BASED PLACEMENT

How Limited Availability Is Managed

When several people are requesting placement in the same home or when availability is limited, PPS may use a need-based review to determine placement priority or order.

Need-based review is intended to organize placement decisions when space is limited. It should not be presented as an automatic denial of housing.
